GRANDMA’S ALZHEIMER FUND

Grandma's Alzheimer Fund was established by the Kobe House Bakery in August 2000. The fund began with proceeds from a fundraising dinner and auction held in memory of Caroline Rozgaj Kobe.

Grandma Kobe treasured her family and proudly watched their accomplishments. Alzheimer’s disease had affected her by the age of 78. This journey was very personal and difficult. She lost the ability to recall the golden memories she’d known so well.

Her family took care of her needs, guided by their instincts. She showed her appreciation in a language not always spoken, but still understood– the language of an Alzheimer’s patient.

Every patient of Alzheimer’s disease has a unique, wonderful life story that becomes impossible to share even with family and friends. Those who must endure its grasp, know there’s little you can do to help.

Grandma’s Alzheimer Fund is an effort to bring comfort to local Alzheimer’s patients in that language written of deeds and not words– as the family of Caroline Rozgaj Kobe remembers with love a special someone they knew as mom and grandma.

AREAS OF INTEREST AND TYPES OF SUPPORT

The grant selection committee takes a special interest in programs that provide opportunities for eastern Jackson County residents diagnosed with dementia. Programs that provide for the health and social service needs of residents are of primary interest.
The grant selection committee prefers to fund specific needs including special programs and projects, capital improvements, and equipment purchases. Multiple year awards will be based on a careful review of a grantee's performance.
Grants are made only to organizations that are tax-exempt under Section 501(c)(3) of the Internal Revenue Code and are classified as "not a private foundation" under Section 509(a).

EXCLUSIONS

The grant selection committee does not fund operating expenses, endowments, individual scholarships, loans, or deficits. The grant selection committee will not fund sectarian religious or political activities, special events, fundraising activities or advertising. Grants are not made to individuals.
The fund representatives reserve the right to modify these guidelines as necessary.

GRANTS RANGE

Grants from the Grandma’s Alzheimer Fund normally range from $500 - $5,000.
 

APPLICATION PROCEDURES

Requests for support may be submitted in a letter addressed to the Foundation office and must include four copies of:
  • a description of the proposed project and the staff that will carry it out
  • a description of the evaluation process for the project
  • a budget for the proposed project, with an indication of amounts already received and other sources of support
  • a list of officers and directors
  • evidence of tax-exempt status under sections 501(c)(3) and 509(a) of the Internal Revenue Code
  • copy of audited financial reports for the previous two years and a copy of the most recent 990 tax form

MEETING SCHEDULE

The grant selection committee normally meets for grantmaking purposes in the Fall. Applications may be submitted at any time but must be submitted prior to September 15th for consideration at the Truman Heartland Community Foundation Grant Award meeting. Applicants will be notified by letter of the decision. Telephone calls to the Director of Programs at the Foundation's office are welcome at any time.

REPORTING REQUIREMENTS

Progress Reports
At the conclusion of the project funded, or twelve months from the date of funding, a written report on expenditure of the grant funds received and accomplishments must be provided to Truman Heartland Community Foundation.   NOTE:  Requests for new funding will not be accepted until final report on previous funding has been submitted.
